Measuring fitness and physical activity in cerebral palsy

[tb] Margaret O´Neil, Kristie Bjornson, Roslyn Boyd, Annet Dallmeijer and Deborah Thorpe were speakers at the conference regarding Fit-Active-Habits on WCPT-congress in Amsterdam in 2011. The symposium was comprised of an international consortium of researchers who conduct measurement and outcome studies to identify effective measures and strategies to improve physical activity and fitness in persons with CP. Tanja Bossmann asked Margaret O´Neil – associate professor at Drexel University in Philadelphia – for the main issues discussed during this focused symposium.

Among others the symposium focuses on measurement methodology protocols on fitness and habitual physical activity outcomes. Could you please summarise the discussed results?

The panel members presented information on their individual studies and the types of measures they use in their research on physical activity and fitness. Panel members presented on either physical activity or fitness measurement protocols. Dr. Kristie Bjornson and Dr. Roslyn Boyd presented their research in which they use accelerometry to measure habitual physical activity in children and youths with cerebral palsy (CP). Bjornson uses a field-based protocol and the StepWatch accelerometer to capture day-to-day physical activity patterns in community and home-based settings. Boyd reported on a lab-based protocol using the Actigraph accelerometer. She established validity of Actigraph in measuring physical activity intensity for youths with CP who walked at different speeds. Both researchers discussed the need to match your instrumentation and measurement protocol to best examine your research question.

Dr. Annet Dallmeijer and Dr. Debbie Thorpe focused on fitness measures for children, youths and adults with CP. Dallmeijer presented on lab-based fitness measurement protocols. She discussed that lab-based measures have high reliability and precision but that they have lower external validity for generalisability and she reviewed fitness measurement protocols for aerobic, anaerobic and walking efficiency. Dallmeijer indicated that we need more measurement methodology studies to determine best protocols in different age groups and levels of CP. Debbie Thorpe presented information on lab measures used to determine fitness changes in community-based fitness interventions for youths and adults with CP. She reported on feasibility and challenges of using arm ergometry in subjects with varying severity of CP.

Could you explain the introduced strategies to enhance health-related fitness and participation in physical activities in children with CP?

Panel participants primarily discussed measurement methodology with limited discussion on intervention strategies. However, Thorpe did present on her current studies to improve fitness in youths and adults with CP using land based (treadmill) and aquatic training programmes. She has completed a grant ...
